From rehabilitation after illness or injury to chronic disease management to in-patient level care, Mass General Brigham’s Healthcare at Home offers patients a broad range of home-based care services within both our Home Hospital and Home Care programs.

Healthcare at Home Programs

Home Care

Mass General Brigham Home Care offers longer-term, post-acute, medically necessary skilled care to help patients manage chronic conditions or recover from an illness or injury.

Visits from Home Care clinicians may be in-person or virtual depending on the patient’s needs and circumstances and cover a large service area. The core clinical care delivery team includes:

  • Registered nurses

  • Licensed practical nurses

  • Physical therapists

  • Physical therapy assistants

  • Occupational therapists

  • Occupational therapy assistants

  • Speech language pathologists

  • Social workers

  • Home health aides

  • Phlebotomists

Home Hospital

One the largest and most established programs in the country, Mass General Brigham Home Hospital provides eligible patients with inpatient-level care in the comfort of their own homes. This service is tailored for acute conditions like heart disease, chronic obstructive pulmonary disease, infection, and others. Rather than being admitted to inpatient units after an emergency department (ED) visit, selected patients who meet the criteria are discharged to their homes, where a team of physicians, nurses, and other providers care for them daily at the same levels as if they were in the hospital.

Visits from Home Hospital clinicians may be in-person or virtual depending on the patient’s needs and circumstances. The core clinical care delivery team includes:

  • Physicians
  • Physician assistants or nurse practitioners

  • Registered nurses

  • Paramedics

Newly Licensed Nurse Residency Program 

The Mass General Brigham's Newly Licensed Nurse (NLN) Residency Program is designed to support exceptional new nurses as they transition from their academics into clinical practice. NLNs accepted into the program will begin their careers in home-based nursing care within a cohort model, divided into North and South regions. Over a 6-month period, they will receive comprehensive education, professional development, and ongoing support.

Within their respective cohorts, NLNs will benefit from mentorship by experienced managers and peer support from fellow NLNs. Through a blend of classes and field work, participants will cultivate critical thinking skills and develop an expert nursing practice.
